About Us

LTS Social Care Services is a growing care-at-home and housing support provider, dedicated to delivering person-centred services that empower individuals, promote choice, and uphold dignity and respect. We are currently looking for a reliable, organised, and experienced Care Coordinator to join our team.

As a Care Coordinator, you will play a vital role in ensuring high standards of care are delivered consistently across our community services. This position involves coordination, supervision, and support of care staff, while also contributing to the effective delivery of holistic care to our service users.

  • Support recruitment, assessments, reviews, and staff supervisions.
  • Supervise and support care staff, ensuring quality service delivery.
  • Assist the Service Manager with day-to-day operations and leadership.
  • Organise and facilitate team meetings, including minute-taking.
  • Liaise with healthcare professionals to support person-centred care.
  • Maintain accurate service user records using Care-For-It or similar systems.
  • Record and report incidents, accidents, and complaints in line with procedures.
  • Coordinate weekly staff rotas and ensure effective staffing levels.
  • Promote and uphold the values of professionalism, trust, and integrity in line with SSSC Codes of Practice.
  • Minimum 2 years’ experience in a home care and supported housing setting.
  • Demonstrable experience in a management or supervisory role.
  • Strong understanding of Health and Social Care Standards and good practice.
  • Excellent communication and organisational skills.
  • Ability to manage time and prioritise tasks effectively, especially under pressure.
  • Commitment to confidentiality and GDPR compliance.
  • Ambition to develop and potentially lead the service.
  • Strong IT skills, including familiarity with care management systems.
  • Knowledge of compliance processes related to recruitment and staff file management.
  • Proactive approach to service development and quality improvement.
  • Ability to attend relevant meetings and workshops to stay current with sector developments.
