Care Coordinator

As a Care Coordinator at LTS Social Care Services, you’ll play a pivotal role in delivering top-notch care in the community. Your responsibilities include participating in assessments and reviews, managing and supervising care workers, setting expectations for each shift, supporting service managers, arranging staff meetings, liaising with other professionals, maintaining records, reporting incidents, and carrying out on-call duties. You’ll also organize weekly rotas and uphold the reputation of the profession according to SSSC guidance.

  • Candidates must possess strong IT skills.

  • ideally hold an SVQ level 3 qualification or be willing to work towards the qualification.
  • A valid driver’s license and access to a car is necessary.
  • Strong communication and interpersonal skills
  • Ability to adapt to various situations
  • Knowledge of PersonCentred and OutcomeBased Approaches

  • Provide leadership to care staff within the service

  • Deliver care according to current best practices and company policies

  • Supervise care delivery within the service, ensuring adherence to standards

  • Maintain current skills through ongoing training and development

  • Participate in on-call duties on a rota basis

  • Ensure service users’ needs and preferences are prioritized for their well-being

  • Support staff and promote efficient service operations

  • Organize and coordinate care delivery, including scheduling and staffing

  • Participate in care delivery, when necessary, especially in the absence of care staff

  • Liaise with other professionals daily

  • Update stakeholders on service capacity and actively seek new clients

  • Interview potential new care staff

  • Collaborate closely with the registered manager to drive business growth  

 

  •  Paid Mileage at 45 pence per mile  
  •  20 days Annual leave plus 8 bank Holidays (oncall shifts).
  •  Minimum Wage Employer  
  •  Salary: £23,000£28,000,
  •  28 days Annual Leave  
  •  Fulltime contract with 39 hours per week (overtime available upon request)  
  • Option for an occupational pension with opportunity to optout
